Vaginal Dryness and Painful Sex After 40

Sex has become painful. Or maybe it's the dryness, the irritation, or the feeling that something down there just changed and nobody warned you. If you're over 40 and experiencing vaginal discomfort, you're far from alone, and there are effective treatments that most women never hear about.

What's happening in your body

The vaginal and urinary tissues are highly sensitive to estrogen. When estrogen declines during perimenopause and menopause, these tissues become thinner, drier, and less elastic. The medical term for this is genitourinary syndrome of menopause (GSM), and it affects up to 80% of postmenopausal women.

Unlike hot flashes, which often improve over time, GSM tends to get worse without treatment. The vaginal walls lose moisture and flexibility. The pH of the vagina changes, making infections more likely. The tissue around the urethra thins, which can lead to urinary urgency and recurrent UTIs.

Many women also notice a decline in sexual desire, partly because of hormonal changes and partly because sex has become physically uncomfortable. This creates a cycle that can be hard to break without addressing the underlying cause.

Signs to look for

  • Vaginal dryness or a persistent feeling of irritation
  • Pain, burning, or discomfort during sex
  • Light spotting after intercourse
  • Reduced natural lubrication, even with arousal
  • Increased urinary urgency or frequency
  • Recurrent urinary tract infections
  • A feeling of rawness or tightness in the vaginal area
  • Decreased interest in sex due to discomfort

What you can do

Over-the-counter lubricants and vaginal moisturizers can offer temporary relief, but they don't address the underlying tissue changes. For lasting improvement, low-dose vaginal estrogen is the gold standard. It comes in cream, ring, or tablet form and works locally, meaning very little estrogen enters the bloodstream.

Vaginal estrogen is considered safe for most women, including many who can't take systemic hormone therapy. It's also one of the most effective treatments in menopause medicine, with most women noticing improvement within a few weeks.

For women who prefer a non-hormonal approach, newer prescription options are available that help restore vaginal tissue without estrogen. A clinician experienced in menopause care can walk you through the options and help you choose what fits your body and your comfort level.

You don't have to accept this as your new normal

Many women suffer in silence with vaginal dryness because they're embarrassed to bring it up, or because a provider dismissed it as just part of aging. It doesn't have to be this way.

GSM is a medical condition with effective treatments. You deserve to feel comfortable in your own body, and getting help for this is no different from getting help for any other health concern.
