Tirzepatide Side Effects and Safety
Tirzepatide most often causes digestive effects, including nausea, diarrhea, constipation, and vomiting. Injection-site reactions, fatigue, and hair loss are also reported with Zepbound. The severity, timing, and effect on eating or drinking help determine what care is needed.

For mild nausea or fullness, smaller portions, slower eating, and less fatty food may help. Drink regularly and ask the pharmacist about persistent constipation. If symptoms interfere with meals, fluids, or daily activity, contact the prescribing team before the next increase.
Persistent pain or fluid loss needs prompt care
Stop treatment and seek immediate assessment for severe or persistent abdominal pain, especially pain reaching the back. Pancreatitis can occur with or without vomiting. Upper abdominal pain with fever, jaundice, or pale stools can indicate gallbladder disease and needs prompt assessment.
Repeated vomiting or diarrhea can lead to dehydration and kidney injury. Inability to keep fluids down, very little urine, fainting, or marked weakness warrants urgent care. Severe swelling of the abdomen with vomiting or inability to pass stool or gas also needs urgent assessment.
Call emergency services for breathing difficulty, swelling of the face or throat, or collapse. Bring the prescription name, dose, and last injection date to any urgent assessment. If an incorrect amount may have been injected, seek medical or poison-control advice immediately.
Hair shedding and injection reactions deserve a review
Hair loss was reported by about 4% to 5% of Zepbound-treated participants across the labeled dose groups, compared with 1% receiving placebo. The label associates these reports with weight reduction. That observation does not identify the cause of every person's shedding or guarantee when it will recover.
Report persistent shedding, patchy loss, or scalp pain and inflammation. The clinician can assess the pattern, recent weight change, nutrition, illness, and other medicines. Avoid starting high-dose supplements without a reason or assuming the prescription must be stopped.
Local redness, itching, bruising, or discomfort can occur at an injection site. Rotate sites and use the instructions for the supplied device. Increasing warmth, spreading redness, fever, or a worsening painful swelling needs assessment for infection or another cause. Widespread allergic symptoms require urgent care.

Other medicines change the risk profile
Insulin and sulfonylureas raise hypoglycemia risk with tirzepatide and may need adjustment. Follow the glucose-monitoring and low-blood-sugar plan agreed with your diabetes team. Symptoms such as sweating, shaking, confusion, or palpitations should be assessed in that context.
Oral hormonal contraception can be less effective during initiation and escalation. The US labels advise a nonoral method or added barrier contraception for four weeks after starting and four weeks after each increase. Discuss pregnancy plans before treatment and contact the clinician if pregnancy occurs.
Keep the team informed about vision changes if you have diabetic eye disease. Tell an anesthesia team about tirzepatide before general anesthesia or deep sedation because delayed stomach emptying can affect aspiration risk. Bring all prescriptions and supplements to the review so interactions can be checked.
