Ozempic vs Wegovy: Same Ingredient, Different Prescriptions

Ozempic and Wegovy both contain semaglutide. Their US labels have different approved uses, doses, and formulations. As of October 2026, both brands include tablets as well as injections, so an accurate comparison needs the brand and route together.

Weight loss can occur during Ozempic treatment, but weight management is not its labeled indication. Wegovy is the semaglutide brand with weight-treatment indications. A clinician considering off-label use should explain the reason, evidence, alternatives, and coverage implications.

The cardiovascular labels also describe different populations. Ozempic requires type 2 diabetes, with further criteria depending on the formulation. Wegovy's cardiovascular indication requires established cardiovascular disease and overweight or obesity. Read the whole indication before treating the products as equivalent.

The dose ranges overlap only partly

Both injections begin with 0.25 mg weekly, but their later doses and treatment targets differ. Ozempic's glucose-control maintenance options are 0.5, 1, or 2 mg. Wegovy adult weight treatment uses 1.7 or 2.4 mg, with a 7.2 mg option for selected adults who tolerate 2.4 mg and need additional reduction.

Ozempic tablets begin at 1.5 mg daily, increase to 4 mg, and may increase to 9 mg for further glucose control. Wegovy tablets also begin at 1.5 mg but progress through 4 and 9 mg to a 25 mg maintenance dose. The matching early strengths do not make their full treatment schedules interchangeable.

Daily semaglutide tablets require an empty stomach, up to four ounces of water, and a thirty-minute wait before food, other drinks, and oral medicines. Weekly injections have separate device and missed-dose instructions. Use the instructions for your actual prescription.

Coverage may differ even with the same ingredient

An insurance plan can cover Ozempic for diabetes while applying different rules to Wegovy weight treatment. Ask about the exact product, formulation, and diagnosis, including prior authorization and renewal. A formulary listing is not confirmation that your proposed use is covered.

A switch needs an explicit prescribing plan

Tell the clinician the current brand, route, dose, last administration date, response, and adverse effects. There is no general milligram-for-milligram conversion from an injection to a tablet. The labels contain specific switching instructions, with some transitions requiring renewed escalation.

The shared ingredient also means shared safety considerations: medullary thyroid carcinoma or MEN2 contraindications, digestive effects, pancreatitis and gallbladder warnings, diabetes-related eye monitoring, and interactions with insulin or sulfonylureas. Semaglutide should be stopped at least two months before a planned pregnancy.

Do not take the two products together or overlap doses to speed a transition. Persistent vomiting, severe abdominal pain, dehydration, or allergic symptoms needs prompt care. The new prescription should include follow-up and the correct administration training.
