Semaglutide Dosage: Product-Specific Schedules

There is no single semaglutide dose schedule. Wegovy, Ozempic, and Rybelsus have different formulations, treatment targets, and instructions. Check the brand, route, and labeled dose on your prescription before using any chart. Dose changes belong with the prescriber.

For selected adults who tolerate 2.4 mg for at least four weeks and need further weight reduction, the current label allows an increase to 7.2 mg weekly. That option is specific to adult weight treatment. Other indications and adolescent treatment have their own maintenance recommendations. If a dose is poorly tolerated during escalation, the clinician may delay the increase for four weeks.

Ozempic injection starts at 0.25 mg weekly for four weeks, then increases to 0.5 mg. For glucose control, maintenance doses are 0.5, 1, or 2 mg weekly. Increases to 1 or 2 mg require at least four weeks at the preceding dose and a need for additional glucose control. The diabetes and chronic-kidney-disease indication has a specified 1 mg maintenance recommendation.

The tablets are taken on an empty stomach in the morning with up to four ounces of water. Wait at least thirty minutes before food, other drinks, or oral medicines. Swallow the tablet whole and take only one per day. Do not split, crush, or combine tablets to construct another dose.

Rybelsus and Ozempic tablets are not substitutable milligram for milligram. Switching between brands, or between oral and injected semaglutide, requires the product-specific instructions and a prescriber's plan. A larger milligram number on a tablet does not mean it delivers an equivalent larger injection dose.

Use the missed-dose instructions for your product

For one missed Wegovy injection, the US label allows taking it as soon as possible if the next scheduled dose is more than two days away. If it is less than two days away, skip it and resume the regular day. Ask the pharmacist if you are exactly at the cutoff. After two or more consecutive missed injections, the current label directs restarting escalation at a lower dose to reduce digestive effects.

For Ozempic injection, a missed dose can be taken within five days. After that, skip it and take the next dose on the regular day. For Wegovy, Rybelsus, or Ozempic tablets, skip a missed tablet and take the next dose the following day. Do not double up.

A long interruption, a switch of formulation, or uncertainty about what was last taken needs direct advice before restarting. The dose you previously tolerated may be unsuitable after a gap.

Milligrams, milliliters, and syringe units mean different things

Milligrams describe the amount of semaglutide. Milliliters describe liquid volume. Syringe units describe markings on a particular syringe. The conversion depends on concentration and equipment. FDA has reported serious errors when compounded products were measured or prescribed incorrectly.

Seek immediate medical or poison-control advice after a suspected overdose. Severe nausea, vomiting, dehydration, or low blood sugar can require treatment. Keep the packaging available so the clinician can identify the strength and amount taken.
